Persistent acne. Unwanted facial hair. Thinning hair. Difficulty becoming pregnant.

On their own, these symptoms may seem unrelated. But together, they can be signs of a condition that affects much more than reproductive health: polycystic ovary syndrome (PCOS), now known as polyendocrine metabolic ovarian syndrome (PMOS).

PCOS has long been associated with periods, fertility, and ovarian cysts, but the condition can also affect hormonal, metabolic, skin, hair, and mental health. Understanding that broader picture can help people recognize symptoms that they may not have realized were connected.

During PCOS Awareness Month, we’re taking a closer look at what PCOS/PMOS can actually look like and why understanding of the condition continues to evolve.

PCOS Is Now Known as PMOS. What Does That Mean?

In 2026, PCOS was renamed polyendocrine metabolic ovarian syndrome (PMOS) following an international consensus process involving patients, healthcare professionals, and professional and patient organizations.

The change reflects something important: this condition is about much more than ovarian cysts. In fact, abnormal ovarian cysts are not required for a diagnosis. The new name is intended to better represent the condition’s broader hormonal, metabolic, and reproductive effects.

Because PCOS is still the term many people recognize, you may continue to see both PCOS and PMOS used as the healthcare community transitions to the new terminology. In this article, we’ll use PCOS/PMOS to help make that connection clear.

What Can PCOS/PMOS Look Like?

PCOS/PMOS can look different from person to person, which is one reason it may be difficult to recognize. Some symptoms can include:

Your periods: Irregular, infrequent, or absent periods are among the most recognized signs of PCOS/PMOS.

Your skin: Hormonal changes can contribute to persistent acne or oily skin. Some people may also develop darker, thicker, velvety patches of skin associated with insulin resistance.

Your hair: PCOS/PMOS can cause increased facial or body hair while also contributing to thinning hair or hair loss on the scalp.

Your metabolism: Insulin resistance is common in people with PCOS/PMOS and can affect metabolic health. Some people may experience weight changes or difficulty managing weight, although PCOS/PMOS can occur across different body sizes.

Your fertility: PCOS/PMOS can interfere with ovulation, which may make becoming pregnant more difficult for some people.

You do not need to experience all of these symptoms to have PCOS/PMOS. The condition can present differently from one person to another.

It’s About More Than Reproductive Health

Periods and fertility are only part of the picture.

PCOS/PMOS can have effects across metabolic, cardiovascular, dermatological, psychological, and reproductive health. The condition is associated with insulin resistance, type 2 diabetes, cardiovascular risk factors, and other metabolic concerns. Depression and anxiety can also be part of the broader health picture.

Having PCOS/PMOS does not mean someone will experience every symptom or develop every associated health condition. However, understanding these connections is important because the condition may require attention beyond reproductive health alone.

This broader understanding is also part of the reason the name is changing from PCOS to PMOS.

Why Can PCOS/PMOS Be Difficult to Recognize?

Imagine two people with the same condition.

One visits a healthcare provider because their periods have become unpredictable. Another has regular concerns about acne and unwanted facial hair. Someone else may first seek answers after experiencing difficulty becoming pregnant.

All could eventually receive a PCOS/PMOS diagnosis.

When symptoms appear in different parts of the body or at different times, they may not immediately seem connected. That’s why looking at the whole picture rather than one symptom alone can be important.

If you are experiencing a combination of irregular periods, persistent acne, increased facial or body hair, scalp hair thinning, difficulty becoming pregnant, or other changes that concern you, consider discussing them with a healthcare provider.

How Is PCOS/PMOS Diagnosed and Managed?

There is no single test that diagnoses PCOS/PMOS.

A healthcare provider may consider your menstrual cycle, medical history, physical symptoms, hormone levels, and other clinical findings. Blood tests or an ultrasound may also be used when appropriate, and other conditions that can cause similar symptoms may need to be ruled out.

Management also looks different from person to person. Depending on someone’s symptoms, overall health, and reproductive goals, care may include lifestyle changes, hormonal medications, approaches to metabolic health, treatment for symptoms such as acne or unwanted hair growth, or fertility treatment when appropriate.

The goal is not simply to “fix” irregular periods, but to consider the individual’s symptoms and broader health needs.

Why PCOS/PMOS Research Matters

Our understanding of PCOS/PMOS is still evolving. The recent name change is one example of how research and patient experiences can reshape the way a condition is understood and discussed.

There is still more to learn about why PCOS/PMOS develops, why symptoms vary so widely, and how the condition affects health over time. Clinical research helps build that understanding and evaluate potential new approaches to diagnosis, treatment, and long-term care.

It can also help ensure that women’s health research reflects the full range of health concerns women experience, rather than focusing only on reproductive health.

Advancing Women’s Health Research at Equity Medical

At Equity Medical, that broader view of women’s health is central to our Center of Excellence for Women’s Health & Gender Equity in Owensboro, Kentucky.

Led by Angela Dawson, MD, a board-certified OB/GYN and experienced Principal Investigator, the center supports clinical research across conditions that affect women uniquely, disproportionately, or differently, including areas such as PCOS/PMOS, endometriosis, uterine fibroids, menopause, cardiovascular and metabolic health, migraine, autoimmune disease, and more.

By bringing specialized women’s health expertise together with clinical research capabilities, our goal is to help expand access to clinical trials and contribute to a better understanding of women’s health across the lifespan.

Learn more about Equity Medical’s Center of Excellence for Women’s Health & Gender Equity:
Explore our Women’s Health Center of Excellence

PCOS, now known as PMOS, can affect much more than periods and fertility.
